Health Worker Survey
Project Goals

The goal of the Community Health Worker (CHW) Survey is to gain a more accurate picture of the CHW profession and capacity in New Mexico, particularly for addressing chronic disease and social determinants of health. Through secondary data analysis, key informant interviews, and surveys, this assessment identified how CHW/Tribal CHR/Promotor(a) are currently being trained, certified, employed, reimbursed, and utilized in New Mexico.

At the Organizational Level:

  • Placement of organizations working with Community Health Workers
  • Types of organizations working with Community Health Workers across the state
  • Health Areas (Issues addressed) by CHW/Tribal CHR/Promotor(a)
  • Reimbursement of CHW/Tribal CHR/Promotor(a) across New Mexico
  • Current practices and barriers related to the integration of CHW/Tribal CHR/Promotor(a) into health and social systems of care, and their use in the community for targeted outreach
  • Technical assistance and training needs (including supervision) of health care systems and community organizations related to CHW/Tribal CHR/Promotor(a)
  • Policies or protocols that incorporate CHW/Tribal CHR/Promotor(a) into health and social systems teams
  • Identified opportunities to incorporate CHW/Tribal CHR/Promotor(a) into chronic disease prevention and management, and addressing Social Determinants of Health
  • COVID-19 Impact on CHW/Tribal CHR/Promotor(a)

This survey is for organizations that employ CHW/Tribal/CHR/Promotor(a)/etc. only

Thank you for taking the time to complete this survey. Your answers will provide a voice for community health workers (CHWs) in New Mexico, and help to support and promote the CHW profession.

All responses will remain anonymous and the information collected will not be connected to you in any way. Your participation in this survey is completely voluntary and you may choose to not participate.

The information collected will help NMDOH Office of Community Health Workers (OCHW) to understand the current workloads and experiences of Community Health Workers across the state. The information will also help OCHW to understand the needs of local CHW organizations and CHWs and to better assist CHWs serve their communities.

This survey will take about 30-45 minutes to complete. All completed surveys are eligible to receive an incentive of $20 discount for a new first year endorsement of CEU Program/Provider Ship status with OCHW, eligible to enter the Certification Lottery, and a certificate of partnership with OCHW. Please contact the OCHW for additional details.

CHW Definitions & Titles

"A community health worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

A community health worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy"
